Fee for Service/ IVF Option Plan
At Reproductive Specialty Medical Center, we understand that the cost of infertility
treatment is very important to you when choosing an IVF center. We work hard to
control the cost of infertility treatment and have developed financial alternatives to
address these concerns. We have designed several financial plans, so you do not
have to sacrifice high quality care when cost is an issue in choosing an IVF Center.
Medical Criteria must be met to qualify for the Option Plan
Fee For Service
$9500.00 (conventional IVF)
$14,500.00 (Donor egg IVF)
$17,500.00 (Donor Egg IVF and Surrogacy)
Included in Services:
*Monitoring of IVF Ovarian Stimulation ( all ultrasound, hormone lab tests, and
physician services for monitoring)
*One egg retrieval procedure
*One embryo transfer procedure
*Anesthesia
*Facility Fee
What is not included, that is an additional cost:
*IVF pre-cycle screening tests Cost can vary
*Trial embryo transfer and hysterosonogram $250.00
* ICSI $1500.00
* Medication costs for yourself & egg donor $3-5000.00
* PGD Pre-implantation genetic diagnosis $4500.00
* Freezing and storage of embryos x 1 yr. $ 500.00
* Egg donor’s fee $ 8-12,000.00
* Any lab tests and ultrasounds after
pregnancy test ( 14 days after transfer)
IVF Option Plan
$15,000.00 (women under the age of 38 using their own eggs)
Included in services
*Two (2) fresh cycles of IVF
*One (1) thawed embryo transfer for cyropreserved embryos
*Freezing and cyropreservation for 1 year
*Anesthesia
*Facility Fee

Medical Criteria to meet for Option program:
Women must not have reached their 38th birthday before beginning the first
stimulation cycle and must be reasonably able to complete the program in the 12
month time limit. Women must complete the medically appropriate pre-cycle testing
and be proven to have normal ovarian reserve, a normal FSH( less than 10 MIUs),
a normal uterus and uterine cavity, no hydrosalpinx, a normal BMI, must be a non
smoker and partner must be a non smoker and must not have previously begun
more than one failed IVF cycle prior to enrollment in the IVF Option plan. Women
with chronic illness (such as diabetes, autoimmune diseases, etc.) must have
medical clearance for becoming pregnant from their physician prior to enrollment.
Men must at least be able to produce adequate sperm for ICSI. Severe male factor
does not qualify for this program. While not every fresh IVF cycle can be expected to
result in surplus embryos of adequate quality for cryopreservation, couples in this
program must agree to have their surplus eligible embryos from their fresh IVF
cycles cryopreserved for future use. Couples must also agree to transfer at least
two embryos per cycle. All pre-cycle testing must have been completed no earlier
than 6 months prior to enrollment.
